RN Case Manager- MATCH (Healthcare)
Healthcare Access Maryland's (HCAM) MATCH Program works with the Baltimore City Department of Social Services to ensure all children in foster care receive health services and their health needs are met. The Registered Nurse Medical Case Manager provides case management for the MATCH program by collaborating with foster parents, parents, and all other members of the interdisciplinary team to coordinate holistic healthcare for children in the MATCH program.
New Sign-on Bonus
The primary responsibilities of this position are:
$3500 with satisfactory completion of the 90 day introductory period

While performing the duties of this job, the employee is exposed to both indoor and outdoor conditions. The employee will be subject to noise.
