At the outpatient centers, we promote overall health and quality of life for individuals, youth and families through evidenced-based therapies and medication management to reduce the impact of mental illness, behavior challenges, and substance use.
Under supervision learns to perform psychosocial assessments, treatment planning/monitoring and discharge planning. Provides counseling/therapy to clients requiring complex intervention. Performs commitment determinations and/or executes orders of involuntary commitments.
Duties & Responsibilities:
Provides biopsychosocial and diagnostic assessments on new, existing clients and/or hospital discharges for those experiencing behavioral health disorders (MH and SA)
Conducts and assists with Crisis Interventions, executing 1013/2013s and collaborating with local hospitals for placement.
Work with clients to develop SMART and recovery-based treatment plans.
Provide short-term individual and group therapy using evidence-based therapies for clients with SPMI &/or SUD
Assist clients in identifying, developing, and sustaining natural support systems.
Provide psychoeducation to clients and families around mental health, substance use, trauma, recovery.
Collaborate with other behavioral health and physical health providers to holistically address client needs.
Complete all required documentation (daily clinical notes, assessments, treatment plans) in an accurate and timely manner (agency standard 24 hrs.)
Participate in regular team meetings to collaborate regarding shared clients, build and practice new skills, and stay up to date on agency administrative information.
Participate in clinical supervision to increase competence and professional development.
Participate in on-call rotation to provide after hour coverage, via phone.
Completes annual training requirements as outlined by agency and/or supervisor.
Performs other related tasks as assigned by the supervisor.
Other duties as assigned.
Minimum Qualifications:
Master's degree in a related field from an accredited college or university AND Fully or Associate licensure (LCSW/LPC/LMSW/LAPC) in the state of Georgia.
Preferred Qualifications:
Experience with Carelogic or other EHR/EMR
Previous clinical experience working in Community Mental Health Care with SPMI, SUD &/or Co-Occurring Disorders
Experience providing ASAM or substance use specific groups such as Dual Diagnosis, DBT, Seeking Safety
Requirements/Competencies:
Must be able to lift 20 pounds.
Requires long periods of sitting/standing.
Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
Candidates for selection must pass a criminal background check (including fingerprinting).
Must pass a pre-employment drug screen and subject to random drug screens.
Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities Note: Some positions may require a valid driver's license.
