The Medical Team, a full-service healthcare organization, is seeking experienced and compassionate anLPNfor our Independent Living facility. The Medical Team provides personal care for seniors in independent and assisted living facilities (such as; showering assistance, toileting assistance, med passing, escorting to and from the dining area, and help dressing and grooming). Essential Functions: Oversee daily activities, manage staff and activities directly related to patient's carePerform or assist in performance of procedures in accordance with physician's ordersCommunicates identified patient needs and any significant changes in a patient's condition consistently and in a timely manner.Provides information regarding patients' rights and responsibilities and treatment plans to patients and/ or family members.Responds to, and communicates, patient questions, concerns, and/ or complaints in a timely and professional manner.Regularly monitor medication cart and perform weekly med cart audits.Participate in arranging and conducting orientation / in-services for nursing assistants/ patient care attendants.Fulfill additional performance responsibilities as assigned and as necessary.
