There is a place for you at Highland Rivers Behavioral Health!Highland Rivers Behavioral Health provides treatment and recovery services to individuals impacted by mental illness, substance use disorders and intellectual developmental disabilities.Are you ready to make a Difference?PROGRAM SUMMARY: The Women's Outreach program (WOR) offers hope, love and assistance for women in recovery by removing substance abuse as a barrier to employment and living in the community. The Women's Outreach program includes both residential and intensive outpatient services that address risk factors for relapse. Services support women in achieving abstinence, maintaining recovery, avoiding illegal activity and meeting parenting responsibilities. The program also provides supports to help women obtain employment and transition into safe and stable housing.JOB SUMMARY: The Substance Use Counselor Provides alcohol and drug assessment, treatment planning, coordinating, and implementing substance abuse counseling activities, and coordinating with other departments, divisions, agencies, and community organizations to ensure the maximum utilization of resources available for chemical dependency treatment and rehabilitationDUTIES ANDRESPONSIBILITIES:
Performs direct individual care as outlined in the HR policy / procedures manual.
Conduct intake assessments on new individuals.
Answers phones.
Performs various administrative, clerical and documentation to support programmatic activities.
Participate in required in-service training and department meetings.
Provide transportation as needed.
Perform urine drug screens.
Facilitate group counseling, group training, individual and family groups.
Maintain assigned caseload.
Perform case management duties such as responding to subpoenas; maintain contact with referral sources and collateral contacts.
Complete all clinical documentation within 24 hours of service provision.
Make appropriate referrals to other community services for additional support as needed.
Develop an individualized discharge planning plan with individuals upon admission to promote a strong recovery and relapse prevention plan upon discharge or completion.
Attend weekly staff meetings.
Complete 30-day review notes on all assigned individuals reflecting the progress towards each treatment plan goal.
Other duties as assigned by Director of Addictive Disease , Program Manager(s), as needed to meet expanding needs of individuals served.
