Case Manager I (292) (R&D)
As part of a service delivery team, provide services to mental health, substance abuse and dual diagnosis consumers.
Maintain clear, accurate and concise consumer records, in a timely manner.
Assistance in improving, maintaining, or restoring an individual or group of individual's functional skills, daily living skills, social and leisure skills, grooming and personal hygiene skills, meal preparation skills, and support resources; and/or medication education
Assist with program design and implementation.
Provide professional linkage/consultation with peers, professionals of other disciplines and outside entities.
Utilize the Fresno Madera Continuum of Care CES policies, processes, and tools
Perform other duties as assigned.
What I bring
High school graduation or equivalent plus two years of related experience, or B.A. degree with no experience in related field from a college or university.
Experience engaging and linking homeless individuals struggling with severe mental illness and/or substance abuse to services.
Class C Driving Requirements
Local resources, information about accessing social welfare systems, advocate on behalf of clients.
Skills to provide case management/linkage services to SMI population.
Ability to relate to others comfortably and be able to establish and maintain effective relationships.
Ability to communicate effectively in writing.
Great people skills, patient and persistent.
Have a positive attitude toward disadvantaged and disenfranchised individuals.
Able to think and work independently to make decisions when faced with daily challenges.
Daily interactions with outreach and clinical staff.
Frequent interactions with people from other agencies.
The case manager works well with others and communicates effectively.
