Housing is an essential component of recovery and independence. View Point Health offers a full continuum of residential services to adults with psychiatric needs and provides stability for individuals experiencing homelessness. The Adult MH Supportive Housing Program is in search of a passionate and skilled Residential Care Provider to become a part of our dynamic team! The Residential Care Provider (RCP) is responsible for oversight of day-to-day functioning of the individuals residing at the home.
The BRIGHT home is a new program planned for implementation in the next 30-90 days and provides intensive support in a residential setting for clients who have a history with high-risk mental health behaviors (self-harm, suicidal ideations, etc.) and Intellectual Developmental Disabilities (IDD) through Dialectical Behavior Therapy (DBT). The individuals are assigned and monitored based upon individualized safety levels of observation. The primary goal for the individuals is to stabilize the self-harm/at-risk behavior through DBT Skills application. The staff will receive training and on-the-job support to help implement DBT Skills and DBT skills review with individuals. The RCP will train and support Independent Living Skills with the individual served in the residence, monitor, and document general activity and progress made of these at-risk individuals.
Duties & Responsibilities:
Clinical Support
Complete Observation Sheet to ensure and document safety levels required for assigned client(s)
Implement and review DBT skills with clients as well as complete daily Skill Review and documentation in Electronic Medical Record
Follow pre-established safety guidelines for each client.
Discuss progress and concerns with supervision for review at Treatment Team Meetings
Independent Living Support
Assist in determining the individuals' strengths, needs, abilities, and preferences by observing daily activities and reviewing records.
Assists individuals in basic problem-solving and daily decision-making using the least intrusive methods appropriate while meeting the safety guidelines.
Assist in providing training and oversight in daily living skills and personal care, such as dressing, hygiene, housekeeping, cooking, laundry, money management and socialization skills using techniques appropriate for their abilities as determined by the treatment team for each client.
Maintains the residence in a clean and organized condition.
Plans, prepares, and serves balanced meals following safety guidelines for each client.
Transports individuals to work, day programs, doctor appointments, shopping, recreational and other activities as needed.
Supervises the self-administration of individuals' medications using agency protocols and maintains required records.
Documents individuals' progress toward goals and objectives identified in their treatment and/or behavior plans through activity/progress notes, data-tracking sheets, and/or other prescribed skill builder formats as needed.
Completes all home and vehicle records, logs, notes, assessments, reports, and other documentation according to protocols.
Lifts and carries groceries and supplies and moves furniture for cleaning.
Assists disabled individuals on/off vehicle and secures seatbelts. May use a gait belt or mechanical lift to lift and position individuals, as needed.
Performs other duties that require moderate lifting.
Minimum Qualifications:
Must have high school diploma or GED AND any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills, and abilities.
Must be 21 years of age.
Must have valid Georgia Driver's License, and a clean motor vehicle report.
Preferred Qualifications:
At least one year of experience working with MH/IDD populations.
At least one year of Specialize High Acuity experience.
Previous experience working in a Psychiatric Hospital setting.
Previous experience working in Mental Health Residential settings.
Requirements/Competencies:
Must pass a pre-employment drug screen and subject to random drug screens.
Must have a valid Driver's License and a clean Motor Vehicle Record in accordance with company policy.
Must pass a criminal background check (including fingerprinting)
Requires TB Evaluation at pre-employment and annually.
Must successfully complete VPH New Hire Trainings, cross train at other VPH MH Homes, DBT Introduction Training and complete 16 hours of training annually
Must be able to lift 20 pounds.
Requires long periods of sitting/standing.
Must have the following technical skills - Word, Excel and Electronic Health Record (Care Logic)
Must be willing to transport residents in the company van.
Must demonstrate the ability to maintain professional boundaries with clients in an intense, specialized residential treatment setting.
Must demonstrate the ability to appropriately redirect challenging behaviors with DBT Skills of clients in an intense, specialized residential treatment setting.
Must be able to lift and carry groceries and supplies and move furniture for cleaning.
Must be able to assist disabled individuals on/off vehicle and secure seatbelts.
Must be able to use a gait belt and mechanical lift to lift and position individuals, as needed.
Assignment of employment will be the BRIGHT (Building Residential Independence, Growth and Hope Together Home) and must be open to other assignments of employment if needed such as Gwinnett, Rockdale, and Newton counties to provide support for clients in the Mental Health Community or to support coverage at another VPH MH home, when needed.
