MDS Nurse (limited term) (Healthcare)

Position: Interim MDS Coordinator (Limited Term)
Location: McKinley Park Care Center, Sacramento, CA

Position Details:

• Compensation:
  • LVNs: $40-$45/hr
  • RNs: $50/hr
• Status: Limited-term opportunity with flexible duration.

Key Responsibilities:

• Assessment and Care Planning:
  
  • Conduct comprehensive assessments (Minimum Data Set - MDS) to evaluate residents' clinical, functional, and psychosocial needs.
  • Develop, implement, and revise individualized care plans in collaboration with the interdisciplinary team.
• Regulatory Compliance:
  
  • Ensure all MDS documentation is accurate, complete, and submitted on time per state and federal guidelines.
  • Stay updated on regulatory changes and train staff on compliance as needed.
• Collaboration and Coordination:
  
  • Work closely with nursing staff, therapists, and other team members to gather data and provide insights into care plans.
  • Lead and participate in care plan meetings and case conferences with residents and their families.
• Reimbursement Oversight:
  
  • Accurately code MDS assessments to support proper reimbursement through Medicare, Medicaid, or private pay sources.
  • Monitor billing practices and collaborate with financial departments to ensure accurate claims.
• Quality Improvement:
  
  • Analyze MDS data to identify trends and areas for quality improvement.
  • Participate in quality assurance and performance improvement (QAPI) initiatives.
• Education and Leadership:
  
  • Provide training and mentorship to nursing and interdisciplinary team members on documentation, assessments, and care planning processes.
  • Be a resource for staff, fostering a collaborative and efficient work environment.
